Routine 2025-06-20 In Violation 6-20-2025 Routine Inspection The person in charge walked with the inspector during the inspection. Food is from approved sources. Certified food protection manager certificate verified. Cold and hot holding temperatures measured within range. Pest control comes monthly. Food storage in coolers and date-marking was correct. Spoke to the person in charge regarding employee health. The person in charge stated they may remove the hand sink that is blocked by shelf. Establishment shall contact our office regarding any changes to the kitchen layout for approval before removing. The inspector spoke to the person in charge regarding cleaning frequency of grease interceptor and is looking into the capacity of the unit. Some small flies were observed in the ware washing and kitchen area. The inspector spoke to the person in charge regarding cleaning drains, treating, and removing sources of attractants. Recheck in one week. Reviewed new code changes and provided a copy of 410 IAC 7-26 to the person-in-charge. Provided placard requirements and two placards to the person-in-charge. These documents are available on the Marion County Health Department: Food and Consumer Safety website. 410 IAC 7-26 code sections that were reviewed include: 7-26-92, 7-26-93, 7-26-25, 7-26-125, 7-26-137-139, 7-26-153, 7-26-201, 7-26-280, 7-26-432, 7-26-185. Routine 2024-08-06 Non-Compliance/Case Closed Single risk factor corrected onsite. Juice label has been submitted to HD as part of plan review and Owner had copy of label on phone. Labels have been reordered, but current batch in customer facing cooler is unlabeled. Discussed adding note to cooler until official labels are received. Recheck 2023-01-26 In Compliance Recheck is in compliance, pest control came and checked for exclusion (rodent access) and the landlord fixed the cove molding and employee restroom. Consumer advisory and labeling present, no recheck needed, thank you. Violations Report
Routine 2023-01-19 In Violation Talked to Owner about violations observed on routine and discussed cooling roasted tomatoes and cooking eggs for menu item, and pest control. >>The tomatoes are sliced thin and cooked in the oven, cooled on a sheet tray on a speed rack and then moved to the walk in cooler into a cambro with a lid on. Reviewed the cooling temperature and times for cooling to identify if cooling time is within cooling parameters of 135 F to 70 F within 2 hours, and 70 F to 41 F within 4 hours. >>Showed the Owner where rodent droppings were found, monitor for activity and owner will be calling for a pest control. >>Certified food manager certification is current. Non-illness Complaint 2023-09-07 In Violation Complaint was justified. Droppings found in hard to reach area under the prep table behind storage bins. Establishment has recently acquired the space next door. Mouse dropping found throughout the new space and in the water heater room of the new space. Person in charge stated that this area has been treated and a trapping was done earlier in the year. Franklin Pest Control is being used. The last service date was 8/14/23. No activity was reported. No standing water was found in the establishment. Side door was being propped open upon arrival. This was corrected. No food was found on the floor but single service items were found to be stored on the floor in the additional space. Plan review application was given to person in charge for the addition next door. Recheck 2023-09-19 In Violation Rodent droppings found in additional room and under dry storage area. Continue to sweep and monitor for rodent droppings. Last pest service report dated 09/13/23 is a monthly service and 1 exterior and 7 interior stations set up with no activity noted on service report. A recheck is scheduled for cleaning follow-up. Non-illness Complaint 2025-01-15 In Violation Reviewed complaint with person in charge. Discussed with employee the procedure if hair were to fall onto an order or a spread and they stated that they discard anything that gets contaminated. When completing inspection observed 3 employees with beards not wearing beard hair restraints. Discussed with person in charge regarding beard hair restraints and they stated that they do not have any policy requiring beard hair restraints. Advised person in charge to add a policy to their employee guidelines regarding hair beard restraints. Violations observed. Recheck set for next week 1/22/25. Violations Complaint Report
