Routine 2025-12-02 In Violation - License Suspended Routine inspection. During the inspection, the dish machine was not sanitizing as required. Inspector tested chemical concentration with test strips and sent thermometer through machine multiple times. No sanitizer was detected. The sanitizer line for the machine appeared to not have any sanitizer running through. Reviewed new code changes and provided a copy of 410 IAC 7-26 to the person-in-charge. Provided placard requirements and 2 placards to the person-in-charge. These documents are available on the Marion County Health Department: Food and Consumer Safety website. 410 IAC 7-26 code sections that were reviewed include: 7-26-92, 7-26-93, 7-26-25, 7-26-125, 7-26-137-139, 7-26-153, 7-26-201, 7-26-280, 7-26-432, 7-26-185. Routine 2025-02-26 In Violation Routine inspection. Kitchen manager walked with inspector. Reviewed menu, approved sources and employee health policy. In-place sanitizer measured 200 ppm quaternary ammonia; compliant. Dish machine measured 50 ppm chlorine; compliant. Hot water at the three-bay-sink measured 123F; compliant. Establishment is on weekly pest service schedule. No evidence of pests were observed in the kitchen or other service areas during the inspection. Floors, walls and ceilings were clean and free of debris. Inspector provided training on cooling and food workers were able to properly demonstrate. Recheck 2022-01-27 In Violation Re-sending Salt block SOPs to inspector, they are complete. Plumber still hasn't made it out, scheduling follow-up visit for three weeks. Chef provided updated certified food handler info, but it is the incorrect organization. Recheck 2022-01-12 In Violation Plumber that they use has been out sick, but they have been in contact with them and they are hoping he can make it out by next week. Please forward written SOPs for using salt blocks in the establishment. SOPs should include the procedures for using the start blocks from receiving to serving - include info like brand and number of salt blocks the establishment keeps on hand, cleaning process, how and where it is stored when not in use, how often you purchase new ones, what items are served on it, and how long the blocks are left to dry before using again, as well as any other relevant information. Routine 2021-12-16 In Compliance PLEASE SEND INSPECTOR UPDATED CERTIFIED FOOD HANDLER INFO. All cold holding and hot holding along the cook line was within range. Soup kettles temp'd at upwards of 165*F. Dish machine was temp'ing at upwards of 150*F - it is set up as chemical. Sanitizer is still working, chlorine test strips are present. Walk-in cooler temp'd at 41*F, prepped food and containers were labeled and date marked. Checked oyster tags - establishment typically keeps them for a year. Dumpster space is shared with the whole building. NOTE: Establishment is currently serving sashimi on a bed of seaweed salad and then placed directly on a block of solid pink salt (used as a plate). Salt blocks are rinsed and stored in walk-in cooler - inspector will look more into salt blocks and what is required for washing/sanitizing and get back with facility. Hood is serviced by KinCo. and is due to be serviced this month. All hand sinks were stocked - plenty of hot water. Checked bar area - coolers within range, chlorine sanitizer packets used - tested well. All alcohol bottles are capped at night with the black plastic caps. Violations Report

Non-illness Complaint 2020-08-06 No Violations Found Spoke with manager - he is aware of potential situations that may have caused the complaint. It is policy for all staff to wear a mask, and there are boxes of extra on site available for staff members. In terms of garnishes at the bar, manager stated it is policy to use tongs to handle the fruit, at which point they stick a plastic pick through the fruit and set it on the glass - no bare hand contact with fruit occurs - also showed inspector on the bar where the garnishes are set up so I could see the station and plastic picks. Bar seating is currently closed. There is hand sanitizer available and signage about masks and social distancing set up. Manager stated masks are required inside at all times, and if staff wants to take them off then they have to take a break and go outside. Violations Complaint Report
Non-illness Complaint 2020-08-26 No Violations Found Observed all employees in the building at the time wearing face coverings correctly, over nose and mouth. Spoke with the Chef who stated he wears his mask while inside during his shift at all times - no matter if he is in the kitchen or the dining area talking with customers. He was working on the evening of the 22nd and doesn't recall any complaints recently. All staff who were arriving for their shifts were wearing face coverings correctly upon time of arrival. Illness Complaint 2024-12-02 In Violation - License Suspended Completed environmental assessment. Reviewed approved sources. Hand sinks were properly stocked and compliant. Sanitizer measured 200 ppm quaternary ammonia compliant. Dish machine measured 50-100 PPM chlorine compliant. Conducted menu review Consumer advisory is in place as required for raw and undercooked product. Oyster shell stock tags were properly maintained and organized for all available and past lots of oysters. Oysters received today were stored properly in the walk-in cooler. Pre-shucked oysters were cold-held improperly in prep-top reach-in cooler. Food worker discarded. Halibut is sourced from an approved supplier. Reviewed procedures with food worker that included proper cleaning and sanitizing of equipment and small wares before and after completing the fish preparation. Portioned halibut measured 40 F on the cookline. Chef drawer door is damaged and does not close correctly. Navy bean salad is a commercially made deli salad held cold and was stored below portioned salmon in the walk-in cooler. food worker corrected. Carrots are sourced from piazza and cooked to order. This product is not hot-held or cooled for later use. Pests were observed in the establishment in equipment and on food-contact surfaces. Establishment shall contact pest provider to schedule additional service to address findings. Establishment shall Clean all of the affected equipment and use monitoring glue boards to monitor further pest findings. Establishment is closed until additional service can be completed and treatment has taken affect. Continue to work with pest provider to eliminate potential harborage opportunities and food sources. Contact a Environmental Health Specialist or the Marion County Health Department following pest treatment to schedule a reopening inspection. A citation has been issued for improper cold-holding. Recheck 2024-12-05 In Compliance Recheck inspection. Pest control has provided treatment and no pest activity was observed during inspection. Pest provider is continuing on weekly treatments and will be back to service establishment on Monday 12/9. Kitchen equipment and facility has been fully deep cleaned and was free of buildup or debris. EHS reviewed with person-in-charge how to properly store small wares. Store utensils in a clean and dry location that prevent potential contamination from debris or buildup. Reach-in cooler and chef drawer unit have been repaired. Recheck 2025-02-26 In Violation Recheck inspection. Establishment has a hole in the wall from fire damage creating a gap on the exterior side of the service hallway.
