This new report and the last report must be provided to the Inspector. (Inspector email left onsite) Professional pest control must be a regular occurrence and Inspector requires documentation stating such. 5. Have a method to monitor pests - A single tin cat trap was observed throughout the kitchen with a glue trap inside. When disposing of the deceased mouse on the glue trap, there were no extra glue traps to replace it to monitor pests and no other traps throughout to monitor activity. There is still standing water on the floor in the dish room from an unknown source. Establishment shall significantly improve conditions and have treatment for small flies performed by 7/03 to avoid further citation. Several pieces of cove moulding have fallen off the walls providing nesting locations for pests. Person in charge is unaware of a plan for the dripping ice machine.
Within 48 hours, develop a pest control plan that includes the following: control of harborage conditions, treatment schedule that includes treatment for small flies, and monitoring plans. There should be significant progress towards controlling the harborage conditions, such as setting up fans to keep the floors dry and having plans to replace the ice machine, by this re-inspection. Citation amount will increase if progress is not made. |

To resume preparing yogurt in house, you will need to submit updated procedures that follow code requirements for cooling and holding. Additional guidelines sent to owner via text message.
Kulfi procedures were not specific enough. Please re-submit kulfi procedures with more detail regarding cooling the milk down to the required temperatures within the required timeframes.

Establishment serves a menu of traditional Indian dishes made with lamb, chicken, fish, and vegetables. No raw or undercooked animal products offered. Majority of food is purchased from US foods, fresh produce is purchased from Fresh Market, and certain spices are purchased from an Indian supermarket in Chicago. All dry goods bear labels with processor information. Dumpster is shared with building, grease dumpster is solely used by establishment.
All sinks in establishment have hot water above 110 deg F. Sanitizer at 3-bay measured 250ppm quat. Food-contact surfaces and equipment are clean to sight and touch. Foods in all coolers measured between 33-40 deg F. Rice hot held in rice cooker measured 137.5 deg F.
Reviewed date-marking and cooling procedures with person in charge and distributed handouts. Please use 3-bay sink until dish machine sanitizer is delivered. Suggest purchasing sanitizer in greater quantities to prevent running out before delivery. Please correct violations before re-inspection. Reviewed sources and cooking procedures with Exec Chef. Food sourced from Restaurant Depot and US Foods. Lentils are washed numerous times and cooked overnight. Skewered chicken cooked in tandoori oven and then microwaved to ensure complete cooking. All foods appeared wholesome and fresh. Temperatures of cooked entree for customer over 165 F and cooked lentils above 165F.

Chicken is received frozen and thawed in the walk in refrigerator. Once thawed it is marinated with lemon juice and backed in the oven. Once cooked it is cut and placed in the cold top or refrigerator.
Rice is cooked and held in a rice cooker. Any left over rice is discarded at the end of the day. The rice was over 150F today.
Naan is a bread with fresh garlic and cilantro.
The hand sink in the kitchen was missing paper towels but immediately restocked.
The dish machine was functioning properly and sanitizer was dispensed at 50ppm.
No employees have been ill in the last 2 weeks or so. An employee health form was left on site.

The walk in refrigerator was at 46-47F. This unit has not maintained proper temperature since the last complaint inspection on August 23. A repairman has been called but has not shown up to make repairs. Do not use this refrigerator for any foods requiring refrigeration until it has been repaired and maintains 41F or less. This includes cheeses, meats, cooked foods etc.
All employees uniforms were clean. No unhygienic practices were observed.
There is a second smaller walk in on site that is maintaining 37F. Any foods that are received must be stored in this refrigerator until the main walk in has been repaired.
All foods in the walk in refrigerator were discarded via trash.
Par cooked chicken was not cooled properly. It was 77-78 after 2 1/2 hours. It was also discarded. Be sure you cool all hot foods from 135F to 70F in 2 hours and from 70F to 41F in another 4 hours.
Do not conduct any catering events until the walk in has been repaired and approved. Management stated there are no catering events for the rest of August. A follow up is scheduled for August 30th. |
