Recheck 2026-01-15 In Violation Monitoring inspection 1-15-2026 Spoke with owner regarding pastry displays and hood ventilation system. Establishment currently has a temporary plan to ensure that there is no risk of food contamination. Establishment is expected to clean the ventilation hood filters due to excessive smoke observed within establishment. The fire suppression system servicing hood is overdue for servicing, with the last documented service in 2016. If establishment wishes to continue cooking bacon and sausage, corrective action must be taken to address ventilation and fire suppression compliance. 30 Day follow-up scheduled. Violations Report
Recheck 2026-01-10 In Violation Follow-up inspection Significant improvement observed regarding pest issue. Pest control continues to come out on a weekly basis. Establishment has plans and materials to fix remaining repairs. Temporary sneeze guard has been added to pastry display. Establishment says they plan on purchasing a proper display case. Recheck 2025-12-12 In Violation Follow-up inspection Pest control came out this past Monday and will continue to come out weekly, no mice observed, some droppings observed. Establishment has shown improvement and progress regarding the recent pest issue. Establishment must have plan for sneeze guard by follow up inspection. If cleaning ventilation filters does not help with smoke in establishment, hoods may need to be serviced. Establishment occasionally cooks sausage and bacon in oven. Follow-up scheduled. Recheck 2025-12-08 In Violation Follow-up inspection 12-8-25 Pest control came out Friday around 3-4pm. All glue traps were changed. Pest control will be back out today and then weekly following todays treatment. All mouse droppings have been cleaned up, floors have been sanitized, and potential openings have been covered. Establishment will continue to work with pest control, and inspector will continue to follow-up with establishment weekly until pest issue is under control. Routine 2025-12-05 In Violation - License Suspended Routine inspection 12-5-25 Establishments license has been suspended due to significant presence of pests. To be reopened establishment must do the following. 1. Clean up ALL mouse droppings throughout establishment; sanitize areas where mouse droppings are found. Old mouse droppings will only lead to more pests. 2. Remove any litter or unused equipment/ items from establishment. 3. Ensure there are no potential outer openings from establishment, none were observed by inspector, however due to the significant presence of pest there is a possibility that there are outer opening hidden throughout establishment. 4. Remove all dead and/or trapped pests. 5. Have professional pest control come out. 6. After 48 hours of professional pest control treatment establishment may contact the following number to have an inspector come out for reopening. 317-221-2248 Please send recent pest control reports to the following email as soon as possible ttownsen@marionhealth.org Establishment must have someone cleaning at ALL times during operating hours after being reopened until issue is under control. During follow-up there will be a discussion on other violations regarding food protection. Establishment's license has been suspended until further notice. Recheck 2025-06-27 In Compliance Recheck inspection. Owner walked with inspector. Front door has been repaired to allow it to close tightly. Leak in mop room has been repaired. Routine 2025-05-02 In Violation Discussed the adoption of the new code and how it effects specifics at establishment. Placard for window and copy of code left onsite with Owner. Establishment does production in early part of the week and has pick up and walk-ins in latter part of week. Menu changes often. Also discussed pest control and sneeze guards/food protection. Initial 2024-06-27 In Violation Establishment is open to customers for carry out and pre orders on Friday and Saturdays, prep takes place on multiple days of the week beforehand. Discussed employee health, building repairs, and pest control with Owner. Pest control managed by Owner and discussed types of food-establishment safe products. Floor in bathroom is planned to be replaced soon, employee access to bathroom has not been affected. Ventilation hood system is present in kitchen over bakery ovens. If intending to use small fryers going forward, please update hood maintenance and move fryers under hood. Until then, remove oil from filters. Owner stated they are testing different menu items and are not serving menu items that require fryer use at this time. Violations Report
Non-illness Complaint 2025-07-18 In Violation - License Suspended Environmental assessment. Manager walked with inspector. Establishment is working on obtaining professional pest control as stated by manager. Establishment has been identifying potential pest opportunities and addressing as possible; as stated by manager. Establishment observed with rodent droppings along baseboards throughout facility. Excessive water and food debris observed on floor in food preparation areas. Establishment's retail food license is suspended. Establishment needs to have professional pest service completed for identified pests and retain invoice and service note for reopening inspection. Establishment shall clean all floors, wall, and other surfaces that may act as food sources for pests and to remove accumulated debris. All non-food contact surfaces and food contact surfaces shall be cleaned. Front and back door need to be tight fitting and close properly to prevent pest intrusion. Ceiling in mop room needs to be repaired to prevent pest harborage. Retain service invoices and other documents for items that cannot be completed prior to reopening inspection to be reviewed. Excessive weeds and debris should be removed around the exterior of the establishment. Remove all unnecessary items from the establishment to prevent harborage and to ensure properly cleaning. Once items are corrected contact the Marion County Public Health Department at 317-221-2222 or your inspector at 317-221-2262 to schedule a reopening inspection. Recheck 2025-07-21 In Violation Recheck inspection. establishment has had pest provider conduct required service and establish a fixed service frequency. Establishment has fully deep cleaned areas, and placed required monitoring traps. Repairs are being conducted in back room area and drywall is being placed to close potential access points by the landlord. Establishment is looking into adding a door with a brush guard to this area as an additional deterrent. Establishment is approved to reopen .
